Cancer screening and prevention tracker: 2026 guidelines, trials, FDA

This tracker logs 2026 developments in cancer screening and prevention. As of 11 October 2026 the main open question is whether the FDA will approve Galleri, the first multi-cancer blood test to reach an advisory panel, which voted in its favour on 23 September.[1][2] Other 2026 milestones include self-collected HPV testing entering US cervical guidelines, a second FDA-approved colorectal blood test, and targeted prostate screening in the UK.[3][4][5]

    What we know

    • NHS-Galleri missed its primary endpoint (stage III–IV) but found 14% fewer stage IV cancers.[7]
    • An FDA advisory panel voted 7-2 (one abstention) that Galleri's benefits outweigh its risks (23 Sept 2026).[1]
    • Galleri was not FDA-cleared or approved as of 11 October 2026.[2]
    • The FDA approved a second colorectal screening blood test, SimpleScreen CRC, on 27 July 2026.[4]
    • The ACS (January 2026) accepts self-collected HPV samples for cervical screening at ages 25–65.[3]
    • The UK NSC recommended PSA screening only for men 45–61 with a BRCA2 variant and family history (May 2026).[5]
    • The USPSTF had not met for more than a year as of September 2026.[11]

    What we don't know yet

    • Whether and when the FDA approves Galleri, and on what label.
    • Whether NHS-Galleri's stage IV reduction holds with longer follow-up and translates into fewer cancer deaths.
    • When the reconstituted USPSTF will meet, and whether it will finalise its pending cervical and prostate updates.
    • Whether USPSTF will add blood-based colorectal tests to its recommendation.
    • Whether Scotland, Wales and Northern Ireland adopt the UK NSC's targeted prostate screening recommendation.

    Timeline data (JSON)

This tracker follows cancer screening and prevention news in 2026: guideline changes, trial results and regulatory decisions. Entries are dated and cited, and the newest appear first in the timeline. For background, start with the cancer screening explainer. Treatment news, including immunotherapy approvals, is covered by the separate cancer immunotherapy tracker.

    Multi-cancer blood tests

The biggest screening story of 2026 is Galleri. In May, GRAIL presented full results of NHS-Galleri, the first randomised MCED trial. It did not reduce combined stage III and IV cancers but found 14% fewer stage IV cancers, a secondary endpoint.[6][7] Some experts said the results were presented more positively than they justify.[8] The full papers appeared in NEJM and Nature Medicine on 22 September.[9] The next day, an FDA advisory panel voted in Galleri’s favour.[1] As of 11 October the FDA had not decided.[2] See the multi-cancer early detection analysis for the competing views.

    Guideline changes

    • Cervical: the American Cancer Society accepted self-collected HPV samples in January, and ACOG endorsed HRSA guidelines with the same option in April.[3][10]
    • Prostate (UK): the UK NSC recommended against population PSA screening and for targeted screening of a small BRCA2 group.[5]
    • USPSTF: no new cancer recommendations. The task force had not met for more than a year, lost its two leaders in May and gained eight new members in September.[11][12][13]

    New tests

The FDA approved Freenome’s SimpleScreen CRC, a second blood test for colorectal screening, on 27 July 2026. Abbott will market it.[4] Its published accuracy is compared with stool tests and Shield in our buyer’s guide.

    Prevention research

A February 2026 Nature Medicine analysis attributed 37.8% of new cancers worldwide to modifiable risk factors, led by smoking, infections and alcohol.[14] The American Cancer Society reported that US colorectal cancer incidence is rising by 3% a year in adults aged 20 to 49.[15] More is in cancer prevention explained.
