New depression treatments: psilocybin, esketamine and the SSRI debate
Esketamine nasal spray is approved for treatment-resistant depression, and a synthetic psilocybin (COMP360) is under rolling FDA review after its developer reported two positive phase 3 trials.[1][2][3] No psychedelic is FDA-approved for depression yet, and independent experts say larger, longer trials are needed.[4][5] Meanwhile a 2026 HHS push to taper antidepressants drew pushback from psychiatrists.[6][7]

Depression treatment is changing on two fronts in 2026. New drugs aimed at treatment-resistant depression are moving through the FDA, and a federal campaign has reopened arguments about the most widely used antidepressants.[2][6] This analysis separates what is established from interpretation and forecasts. It is not medical advice; do not start, stop or change any treatment without a clinician, and do not use unregulated psychedelics on the basis of this page.[11]
The problem new treatments target
NIMH defines treatment-resistant depression (TRD) as no improvement after trying at least two antidepressants.[12] Compass Pathways, a psilocybin developer, estimates that about 4 million people in the US with MDD have TRD.[13] Established options for TRD include add-on medicines, ECT, which NIMH calls the “gold standard”, and rTMS.[12][14]
Esketamine: the rapid-acting precedent
Esketamine nasal spray (Spravato) is FDA-approved for TRD in adults and, since January 2025, can be used without an oral antidepressant.[1][15] NIMH says it typically acts within a couple of hours.[16] Because of risks of sedation, dissociation, respiratory depression and misuse, it is given only in certified settings under a restricted program, and it carries a boxed warning.[17][18]
Esketamine matters for the psychedelic debate because it established a supervised, in-clinic delivery model for a fast-acting drug with abuse potential. Any approved psilocybin product would probably need similar restrictions, which shapes cost and access as much as efficacy does.
Psilocybin: what the evidence shows
A key peer-reviewed psilocybin trial, a phase 2 study in the NEJM in 2022, randomised 233 adults with TRD to a single dose of synthetic psilocybin (25 mg, 10 mg or 1 mg as control) with psychological support.[19] At three weeks the 25 mg group improved more than the control (a 6.6-point MADRS difference); the 10 mg group did not, and sustained response at 12 weeks was not clearly supported.[20] Adverse events occurred in 77% of participants, and suicidal ideation, behaviour or self-injury occurred in all dose groups.[21] The trial was funded by the developer.[19]
Compass reports that COMP360 met its goals in two phase 3 trials, COMP005 and COMP006, with more than 1,000 participants across its TRD programme.[3] By the company’s account, 39% of COMP006 and 25% of COMP005 participants had at least a 25% reduction in MADRS scores at week 6, maintained on average through week 26.[22] These figures are the company’s own summaries; we have not seen independent analyses of the phase 3 data.[3]
The company’s chosen responder threshold (a 25% score reduction) is lower than the 50% reduction the NEJM trial used to define response, so the two sets of numbers are not directly comparable.[22][23] Independent analysts at Penn’s Leonard Davis Institute describe the evidence as promising but limited.[5]
Regulatory status, October 2026
In April 2026 the FDA granted Compass a rolling NDA submission and selected COMP360 for a Commissioner’s National Priority Voucher, which can shorten review to one to two months after filing.[2][24] Separately, Executive Order 14401, signed on 18 April 2026, directs such vouchers to psychedelic drugs with Breakthrough Therapy designation, sets up a Right to Try pathway and allocates at least $50 million for state partnerships.[25][26] The order asks for a prompt rescheduling review of Schedule I products that complete phase 3, but it did not change psilocybin’s legal status.[27][28]
Compass expects to complete its application in the fourth quarter of 2026 and to launch in the first half of 2027, subject to FDA approval and DEA rescheduling.[2][29] With a priority voucher, an FDA decision in early 2027 is plausible, but the timeline depends on the filing being complete and accepted, and on any advisory committee. We would treat approval as likely but not certain, and broad access as further away because of rescheduling, state rules and clinic capacity.
The 2026 antidepressant controversy
In May 2026 HHS Secretary Robert F. Kennedy Jr. announced policies to encourage clinicians to help patients taper off antidepressants such as SSRIs when appropriate, including training, clinical guidance and billing changes.[6] HHS’s letter to clinicians said psychiatric medications can play an important and at times essential role.[30] Under the plan, SAMHSA is to publish prescribing data and guidance, and CMS is to issue billing guidance and expand access to talk therapy.[31] An HHS spokesperson said there had been no discussions about banning SSRIs.[32]
American Psychiatric Association leaders called the overprescribing framing an oversimplification and said the main problem is access to care.[7] The APA’s chief executive said Kennedy’s past claims that SSRIs cause violence, addiction and withdrawal worse than heroin are false.[33] The APA also said it supports better training on safe tapering, and its president joined an HHS panel on tapering guidance.[34] About one in six US adults report taking an antidepressant.[35]
There is more agreement here than the headlines suggest. Both HHS and the APA endorse informed consent and supervised tapering where appropriate.[30][34] The dispute is about emphasis and messaging: whether the main risk is overuse of medicines, or people stopping effective treatment abruptly, or never getting care at all.[7][36] For patients, the practical advice from NIMH is unchanged: talk to a clinician before stopping any medicine.[11]
Watch for SAMHSA’s promised prescribing data and tapering guidance, and for any FDA label reviews. If guidance emphasises gradual, supervised tapering, it is likely to align with existing clinical practice; if it questions SSRI effectiveness broadly, expect further professional opposition.

A new class for hard-to-treat depression
Treatment-resistant depression leaves many people without relief. Esketamine showed rapid-acting drugs can reach patients through supervised clinic models, and psilocybin's developer reports benefit lasting months after one or two doses. Federal policy is now actively speeding review.[16][22][24][26]

Questions readers ask
Is psilocybin FDA-approved for depression?
No. As of October 2026 no psilocybin product was approved. Compass Pathways' COMP360 was under a rolling New Drug Application, with the company aiming to complete it in the fourth quarter of 2026.[4][2]
When could psilocybin therapy be available in the US?
Compass says it expects a launch in the first half of 2027, but only if the FDA approves it and the DEA reschedules psilocybin. That is a company forecast, not a certainty.[29]
What did the 2022 NEJM psilocybin trial find?
In 233 adults with treatment-resistant depression, a single 25 mg dose reduced depression scores more than a 1 mg control at three weeks, but not clearly at 12 weeks. Side effects affected 77% of participants.[19][20][21]
Is the government banning SSRIs?
No ban was announced. In May 2026 HHS launched a plan to encourage tapering when appropriate, and an HHS spokesperson said there had been no discussions about banning SSRIs.[6][32]
What did the April 2026 executive order on psychedelics do?
Executive Order 14401 told the FDA to give priority vouchers to eligible psychedelic drugs, set up a Right to Try pathway, funded state research partnerships and asked for a fast rescheduling review after phase 3 success. It did not change the drugs' legal status.[26][27][28]
